Summary:
A Community Support Specialist is needed for ICORT Team for Forrest County. This position will provide case management services to adult participants on the ICORT Team. Some on-call duties required. ICORT is a program that serves individuals with severe and persistent mental illness in the community.
The Intensive Community Outreach and Recovery Team (I-CORT) is a recovery and resiliency-oriented, intensive, community-based rehabilitation and outreach service for adults with a severe and persistent mental illness. I-CORT is a team-oriented approach to mental health rehabilitation intervention and supports necessary to assist individuals in achieving and maintaining rehabilitative, resiliency, and recovery goals.
I-CORT services are provided primarily in natural settings and are delivered face-to-face, by phone or video conference with the individual and their family/significant other as appropriate, to the primary well-being and benefit of the recipient. I-CORT assists in the setting and attaining of individually defined recovery/resiliency goals. I-CORT primary treatment objective is to assist in keeping the individuals receiving the service in the community in which they live avoiding placement in state-operated behavioral health programs.

Requirements:
Bachelor's degree in behavioral healthcare required
Must have reliable transportation to provide community services
Must have a valid driver's license with a satisfactory driving record
Must be able to obtain DMH case management certification within 60 days of hire
Must have auto insurance liability limits of 100,000/300,000/50,000 within 1 month of hire
Behavioral healthcare experience preferred
Excellent communications skills required
Responsibilities:
Assists with medication monitoring and system navigation.
Assists with outreach of program and case management.
Demonstrates knowledge of diagnosis, signs and symptoms of relapse, medication information, and knowledge of community resources to explore all alternatives to hospitalization.
Facilitates Person Centered Planning meetings.
Provides advocacy, consultation, and educational services.
Provides outpatient social services linkage, emergency intervention, and crisis intervention services.
Provides services to clients through home or community visits.
