This Position is for a Social Services Program Coordinator III with an agency location in Long Beach MS.

Summary: Serve as the Support Coordinator (SC) for people enrolled in the Home and Community-Based ID/IDD Waiver Program. Manages and maintains caseload of people receiving ID/IDD Waiver Services. Develops and maintains a person-centered Plan of Services and Supports (PSS) for each person on caseload. Revises/updates each person's PSS at least annually or when changes in the person's circumstances occur or when requests are made by the person/legal representative. Coordinates and monitors identified supports and services through regular monthly contacts, face-to-face quarterly reviews, and annual assessments and re-certificatioin meetings. Ensures services are adequate, appropriate and meet the person's needs Travel throughout the six southern counties will be required.

Qualifications and Requirements: Typically requires a Bachelor’s Degree in a related field and 4-7 years experience adn one (1) year of experience working with poeple with IDD. Must be eligible to drive a Satate of MS vehicle.
